OUR LEGAL DUTY

West Vision Care, PLLC is required by law to maintain the privacy of your Protected Health Information, referred to as PHI. We are required to provide you with this Notice of our legal duties and privacy practices. We will follow the terms of the Notice that is currently in effect.

COMPLAINTS

If you believe your privacy rights have been violated, you may file a complaint with West Vision Care, PLLC or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
